Cheques and Forms

The Parent's meeting has been moved to the evening of September 5th because too many people are away on vacation at this time.  For this reason all players must bring the two cheques and 3 forms to the coaching staff as soon as possible.  Here is a complete list of what is required:

• Medical form  (download here)
• Code of conduct form  (download here)
• Registration cheque for $375 (can be post dated to Aug 31st)
• Gear deposit cheque for $150 (will be returned when gear is returned)
